Virtual wards

Virtual wards allow patients to get the care they need at home safely and conveniently, rather than being in hospital. The NHS is increasingly introducing virtual wards to support people at the place they call home, including care homes. Luscii supports virtual wards and has a range of virtual wards readily available. Our platform supports 50+ care pathways already.

Luscii virtual wards available for
  • Respiratory conditions (COPD, Asthma, Covid-19)
  • Heart Failure
  • Palliative care
  • Frailty
  • Pediatrics

What is a virtual ward?

NHS England defines virtual wards as "a safe and efficient alternative to bedded care that is enabled by technology. Virtual wards help patients who would otherwise be in hospital to receive the acute care, monitoring and treatment they need in their home.

Virtual wards support early discharge of patients and prevent (re-) admissions. This video illustrates how Wolverhampton ICS has implemented different virtual wards.

Why set up a virtual ward?

A virtual ward has benefits for patients, clinicians & nurses and the health system as a whole.

  • Improved long-term management of diseases
  • Better and co-ordinated multi-disciplinary team support during acute phases of illness
  • Reduced complication through reduced length of stay
  • Less hospital admissions
  • Early supported discharge
  • Better, equity of patient outcome across your region
  • Better patient experience through clinician support at every stage of their care pathway.

Guidance & Funding

The NHS 2022/23 priorities and operational planning guidance includes the request to Integrated Care Systems to maximise the roll-out of virtual wards. NHS England expects ICSs to have developed virtual wards with the ambition to 40 - 50 virtual beds per 100,000 population, at least for acute respiratory infection and frailty

Funding is available through the Primary Care System Development Fund (SDF) totaling to £450m (£200m for FY 22/23 and £250m for FY 23/24).

Flexibility

The Luscii solution is flexible, type of measurements and educational material can be tailored to create unique programs to work for your specific clinical service and patient cohort.

Scale

Scale to more virtual wards and beyond to ensure system wide readiness. Experience the benefits of collaborative virtual monitoring being spread beyond the acutes into community and self care models. Luscii also supports patient initiated follow-up (PIFU) in various pathways.

Benefits

The benefits to the patient are improved long-term management of diseases,
better and co-ordinated multi-disciplinary team support during acute phases of illness.
